相傳在唐朝時期，長安有一對孤兒寡母，丈夫因患肺癆而去世了，母子二人孤苦伶仃，以行乞謀生，城里有個開米粥鋪的何氏夫妻收留了他們，把母子倆留在粥店幫忙，何氏夫婦還經常把每天剩下的粥救濟給那些沒飯吃的窮人，百姓都說何氏夫妻是大好人，一定會好人有好報。

這一件件善事都被觀音菩薩天耳所聞，天眼所見，因此觀音菩薩化身為一位王公貴族，賜何氏夫妻一件碧玉如意，保佑他們事業興旺，無病無災，修習祈禱，延年益壽。這就是葉衣觀音的故事。
